Do you have diabetes?
Are you passionate about local people being able to manage this condition more effectively and receive better care?
Then would you be interested in joining a quarterly meeting group as one of our patient representatives? Helping to advise health professionals who make decisions about the future care and education offered locally for people living with diabetes?

The North East Hampshire and Farnham Clinical Commissioning group or CCG, is responsible for planning and monitoring health services locally. We need to know it’s working for the people who use those services and often invite them to join us as we try to work out the good from the bad and where we can improve.

We are currently looking for people who live with diabetes to join our quarterly meeting group, which takes place at Aldershot Centre for Health on a Tuesday afternoon between 3.30pm – 5pm. The group is made up of health care professionals and local people who have an interest in or live with diabetes. We really want to focus on how individuals can better control their blood pressure, cholesterol and HbA (or glycated haemoglobin).

As the voice of the person living with diabetes you are there to offer your own lived experiences, comment on what has worked (or not worked) for you, highlight how well you understand your own condition or what information you feel would benefit you and help to come up with new ideas or initiatives that could help others manage their condition. We are really looking for someone who is honest but prepared to be positive and help find the solutions if it’s not all good.

Expenses are paid to cover mileage and parking.

Skills

Have either type 1 or type 2 diabetes.
Good communication skills.
A positive attitude to helping facilitate changes in services.
Available to attend a quarterly meeting within the working week.
